Registration information for Spring 2011 season.
There are two methods to register for the spring 2011 season:
1) The new PointStreak online website registration process - create an account, register player, bring payment (cheque, cash or money order) to first practice
2) For the “mail in” registration
Print out a copy of the registration form from the website, complete all sections that apply, have both the player and parent/guardian sign the form. Fees may be paid by cheque* or money order (payable to Fundy Minor Football, no postdated cheques please). Include proof of age (photocopy of Medicare / birth certificate / photo ID card) and mail to:

This is our first time for an online registration. Please encourage players to ‘pass the word’ to friends and class mates who may be interested in playing this spring.
* There will be an additional $25 fee charged for NSF cheques (beyond your bank fee).

Registration
information for Fall 2010 season (To be updated soon)
For the “in person” registration (August 15th to season
start)
Print out a copy of the registration form from the website, complete all sections
that apply, have both the player and parent/guardian sign the form. Obtain a
proof of age document (photocopy of Medicare / birth certificate / photo ID
card) and bring both documents with you to the practice in your zone (check
website for information / practice times / coaches contact information). The
registration fee for this registration is $20 more than for the mail in registration
and must be paid to the FMFA contact person directly (no cheques for registration
after August 15th, cash or money order only).
Family Discount
For FMFA families with multiple players,
the highest registration fee will be paid in full, with additional siblings
receiving a $25 reduction in the fall registration fee.
Refund Policy
The executive of Fundy Minor Football recognizes that on occasion a situation arises that a registered player decides not to, or is unable to play football.
If a registered player decides not to play (for any reason) prior to the season start, whomever paid the registration fee will be entitled to a refund of the registration fee less $25. The parent / guardian of the player who is seeking the refund must contact the registrar directly (coaches are not authorized to handle refunds). All refunds will be in the form of a cheque and no refunds will be processed until all football gear is returned. Due to the very busy period in September, no refunds will be processed until the month of October.
Once a player has participated in a regular season game no refunds will be made.
Children’s Fitness Tax Credit
The fall programs offered by the Fundy Minor Football Association fulfill the requirement under the income tax act to qualify for a tax credit. Income tax receipts will be issued for all registered participants in the name of parent / guardian who pays the registration fee. These receipts will be available from the coach at the end of the season upon the return of all the football gear in good order. Duplicate receipts will not be issued.
Insurance
Fundy Minor Football as a member of Football New Brunswick is covered by an insurance policy provided through Football Canada. The intent and focus of this policy is to protect the association, and its volunteer members against claims resulting from bodily injury or general damages for which they could be held legally liable. This policy also includes specific limits of insurance payable for injuries / accidents. This portion of the policy is very limited in scope and amount of payment. Although Medicare will cover most hospital care there are a number of items that it will not cover (for example some braces for soft tissue injury). Most families have supplemental coverage for these items through workplace insurance (Blue Cross etc.), however you should review your coverage and decide if additional supplemental insurance is warranted. Fundy Minor Football will not be liable for additional cost resulting from injury or rehabilitation. The policy runs from August 1st to July 31st and is subject to revision.
Financial Assistance
Fundy Minor is proud of its history of providing quality recreational opportunities for youth at a reasonable cost. We are the only major (geared) sport that provides all equipment for the players. However we recognize that there are times when parents / guardians cannot afford our modest registration fees. We want all players who wish to play to have the opportunity to do so. We welcome the funding organizations such as P.R.O. Kids and Kidsport which provides a third party review of the financial situation and funding for our registration fees. These funding groups do require lead time to process applications and we strongly urge person who are planning to apply, to do so early.
